**St. Mary's Home care & Hospice is seeking a full time Hospice RN Case Manager**
*****$10K Sign on bonus offered******
*****Signing Bonus is not applicable to internal employees, former employees who have resigned in the last year or applicants sourced from outside firms.*****
**Hospice RN Case Manager position summary**
**Responsible for case managing a team of hospice patients. RN case manager will collaborate with interdisciplinary team and attend weekly Interdisciplinary meetings. RN Case manager will interact with the primary care physician and/or hospice medical director as needed to effectively manage patient symptoms. Position is full time primarily Monday-Friday. No on call or weekends.**
