In this role, you will support patients and families during one of the most meaningful phases of care. You will collaborate closely with the interdisciplinary team, manage individualized care plans, and ensure timely, accurate documentation. Required Qualifications
- Associate degree or higher
- 2+ years of Hospice experience
- 1+ year of experience in ANY of the following: Electronic Medical Record (EMR) Other EMR systems TruChart EHR
