Health Services Coordinator

POSITION TYPE: FULL-TIME

Responsibilities

• Facilitate communication and collaboration among nurses/participants/staff for our drop‑in programs
• Establish and coordinate relationships with other healthcare services/external agencies for program development and implementation
• Coordinate care plans and services with external partners
• Research and identify potential resources and grants for health care
• Prepare and submit grant proposals to secure funding for healthcare programs
• Adhere to PHIA/FIPPA regulations and maintain strict confidentiality of participant information
• Facilitating Health Programs: Engaging and Presenting to Participants
• Organize and maintain participants’ medications, ensuring all prescriptions are current and accurately documented on MAR (Medication Administration Record) sheets. Dispose of any unused medications after two weeks, following program protocols
