Job Description
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team at a Nursing & Rehab Center. The LPN will provide high-quality nursing care to residents, support daily medical needs, and ensure a compassionate, resident-centered approach to healthcare.
Key Responsibilities:

• Provide direct nursing care to residents, including medication administration, wound care, and vital sign monitoring.
• Collaborate with RNs, CNAs, and other healthcare professionals to develop and implement individualized care plans.
• Assist residents with daily living activities and ensure their comfort and well-being.
• Document patient progress, report changes in condition, and communicate effectively with the healthcare team and families.
