Geriatric Care Coordinator

Timmins and District Hospital is currently seeking a Geriatric Care Coordinator for the Emergency Department. Unit. Reporting to CCU/ER, Acute Medical Director, and The Geriatric Care Coordinator will collaborate with members of allied health team and external partners to ensure high quality, effective and efficient services and care is provided to the aging population. Their focus will be on those frail seniors who are at risk of suffering adverse events, loss of independence and admission to hospital or long-term care. Geriatric Care Coordinator will provide expert and targeted assessment and treatment services in the ED; they enhance linkage with hospital and community services and are advocates of the seniors they serve.
