Become a crucial part of VON Canada as a Full-Time Case Coordinator in Ontario. Guide clients through comprehensive assessments and create personalized care plans in a hybrid work model.
This regular position focuses on delivering high-quality service through assessments and care planning. You will work both from home and in the community to support clients. A strong background in health or social services is essential, as well as experience in case management.
