Job Description
Service Care Solutions are seeking an experienced Advanced Clinical Practitioner (ACP) to join an innovative and established Frailty Team . You’ll work autonomously at an advanced clinical level, delivering person-centred care to frail and vulnerable patients in their homes and communities, supported by a multi-disciplinary team.
Location: Scarborough and surrounding areas
Contract Type: Fixed Term – 24 Months
Salary: Competitive (Per Session, based on experience)
Hours: Full-time / Part-time considered
Reports to: Director of Community Services Based in our Scarborough, you will provide leadership in delivering holistic care that helps reduce hospital admissions and facilitates early discharges for complex patients.
