**Fairview Health Services** is seeking a **Family Medicine Physician** to join our established primary care practice at **M Health Fairview Clinics – Oakdale** in Oakdale, Minnesota. This is an excellent opportunity to join a collaborative, multi-specialty clinic.
**Practice details**
+ Established primary care clinic with **11 provider primary care** practice which includes 9 Family Medicine Physicians and 2 Nurse Practitioners.
+ Average patient volume of approximately **16–24** patients per day
+ As part of Fairview’s Accountable Care Organization (ACO), providers practice in a collaborative, team-based model focused on quality outcomes, care coordination, and population health management.
+ **Opening: 1.0 FTE (34 patient contact hours) - flexible schedule.**
+ Fully integrated electronic medical record (Epic)
