The successful candidate will play a key role in delivering high-quality patient care, supporting chronic disease management, and promoting health and wellbeing within our local community and supporting the wider multidisciplinary team.

The successful candidate will play a key role in delivering high-quality patient care, supporting chronic disease management, and promoting health and wellbeing within our local community and supporting the wider multidisciplinary team.
Main duties of the job
- Deliver high standards of nursing care to patients within the practice setting
- Undertake chronic disease management including asthma, COPD, diabetes, and hypertension reviews
- Carry out cervical cytology (smears) in line with national screening programmes
- Administer immunisations and vaccinations (including childhood and travel vaccines)
- Perform wound care, dressings, and removal of sutures
- Conduct health promotion and lifestyle advice clinics (e.g. smoking cessation, weight management)
