Oldham Early Intervention Team are seeking to recruit a band 6 Care Coordinator to join the multidisciplinary team.

We work with people experiencing a first episode psychosis or at risk of developing psychotic symptoms, providing assessment, treatment, psychological therapy, physical health monitoring and family and carer support.

Main duties of the job

To act as Care Coordinator for service users identified as having a first episode psychosis, being in the early phase of a psychotic illness or at risk mental state.
