University Hospitals Bristol and Weston NHS Foundation Trust (UHBW) invites applications for a Discharge and Flow Coordinator – a key member of the Transfer of Care Hub supporting patients’ discharge planning from admission through to community care. The role focuses on working collaboratively across multidisciplinary teams to ensure patients have a smooth transition home.

Main duties of the job

• Act as the first point of assistance for internal and external enquiries regarding patient discharge.
• Facilitate and support early communication between hospital staff, patients, and families related to discharge planning.
• Manage patient communications via telephone and Trust systems, creating letters and emails.
