KEY FUNCTION

To provide comprehensive nutritional assessments and care to complex medical patients, with a specific focus on the older population.

REPORTING

Under the general direction of the Clinical Manager, Acute and Reactivation Care Unit.

DUTIES

1. Interpret anthropometric, biochemical, physiological data in relation to nutrition status.
2. Calculate patient’s daily nutrition requirements (macro- and micro-nutrients including energy, protein, vitamins, minerals, fluids, etc.) based on available data.
3. Provide Enteral and Parenteral Nutrition support, lab interpretation and correction.
4. Provide therapeutic counselling to patients and/or family/caregiver(s) on an individual basis.
5. Monitor patient progress and liaise with the multidisciplinary treatment team and other relevant health agencies (i.e. Community Care Access Centre).
6. Complete nutritional supplement prescriptions as required...
