Diabetes Educator

The purpose of this position is to provide nutrition counselling, therapy, and education for patients with diabetes. As part of the diabetes team, the position is responsible for educating referred clients (encompassing the individual, family, and community) about the physiology and treatment of diabetes, enabling clients to make informed decisions about their health.

The Registered Dietitian will work closely with other members of the diabetes team to provide diabetes care and education to clients and families ensuring consistency within the provincial program. This is performed through nutrition counselling, assessment of individuals, development and ongoing monitoring of care plans for self‑management. This position is located within the Primary Care Networks and will support patients provincially as needed. This position may work occasional evenings and weekends as required.
