OSF ITEMS OF NOTE:
- Local Rate: Applies to clinicians with permanent addresses within 50 miles (-*** to regular and OT bill rates)
- OSF Call Off Policy: Equivalent of 12 hours every 13 weeks with no penalty or need to pay for the shift
- Non-billable Orientation Hours: 12 hours, guaranteed hours will not apply during orientation
REQUIREMENTS FOR SUBMISSION:
- Last four (4) digits of clinician's SSN
- Clinician's middle initial
- Requested time off
- Active IL license or confirmation that application process has been initiated
- Your candidate understands OSF is a Drug-Free Campus and they must produce a negative drug screen for all panels

- General Certification Requirements: Would depend on the modality needing a traveler. BLS, Illinois license for radiation and Certification depending on the modality.
