Job Description
**Job Description:**
The Certified Mental Health Professional utilizes clinical expertise to perform psychosocial assessment and provides quality behavioral health services including being responsible for treatment plan development, coordination, and the delivery of psychotherapy to patients and families. This position works collaboratively with patients, patients' family/significant other, healthcare providers, insurers, community resources, and other involved parties to identify patients' needs, broker services, coordinate care and transition plans, and advocate for patients.
This care is provided under the supervision of a licensed Clinical Mental Health Counselor (CMHC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).
