About The Opportunity

The Care Coordinator is responsible for the assessment, care planning, authorization of services, and ongoing case management of clients referred to Continuing Care. Care Coordinators help provide an array of services and programs offered by Nova Scotia Health Continuing Care and the Department of Seniors and Long Term Care to assist individuals and families in managing complicated acute or chronic health situations in the community. They assist clients to identify goals, unmet needs, and resource requirements, and based on the assessment, formulate a plan to meet these goals.
