Your new role
As a Complex Needs Specialist Practitioner, you will provide holistic case management to families of children with complex disability needs, supporting safety, wellbeing, and stability. You will undertake therapeutic assessments, develop individualised Family Action Plans, and assist families to navigate the NDIS while advocating for appropriate supports.
Working collaboratively with multidisciplinary professionals, you will implement behaviour and parenting strategies, coordinate care teams, and broker services aligned to family needs.
