Key Responsibilities:
• Deliver medical care to long-term care residents
• Manage chronic co-morbidities in collaboration with MDs

Requirements:
• Completion of a recognized Nurse Practitioner program
• Current certification with College of Nurses of Ontario
• Experience in chronic disease management preferred
