Job Description

CenterWell Home Health is seeking a Home Health LPN to provide one-on-one nursing care in patients’ homes under RN supervision. You will implement care plans, educate families, and document care activities as part of a coordinated team.

The role requires an active nursing license in the practicing state, CPR certification, and at least two years of LPN/LVN experience in clinical settings. A valid driver's license and reliable transportation are also required.
