Community Support Worker

Brother Bear Support Services Pty Ltd is seeking a motivated and compassionate Community Worker to join our growing NDIS team on a full-time basis in Thomastown, Victoria.

The successful applicant will work collaboratively with participants, families and service providers to promote independence, community inclusion and capacity building through person centered planning and coordinated supports.

1. Assess participant needs and assist in developing individual support and community participation plans.
