Community Support Worker

Assist individuals with their physical, economic, vocational, recreational, social, emotional and daily life skills development. The Community Support Worker assists individuals to achieve the highest quality of life possible. Promotes and upholds the rights of individuals we serve and honours their choices in accordance with the policies and procedures of the agency.

Key Duties and Responsibilities

• Participates in assessment, goal setting and program planning such as Individual Service Plans for individuals living in the community. Documents and implements the plan. Provides input into the evaluation of the program.
