Community Support Worker

Care Fast Group is a registered NDIS provider supporting people with disability to live safely, meaningfully and as independently as possible in their homes and local communities. We are seeking a full-time Community Support Worker to join our team and provide practical, person-centred support to NDIS participants with varied support needs, including participants living in supported accommodation and community-based settings.

This role is suited to someone who can work beyond routine care tasks and actively contribute to each participant’s community participation, daily living goals, wellbeing, safety and connection with appropriate services.

About the role

As a Community Support Worker, you will work directly with participants, families, support coordinators, allied health professionals and internal Care Fast teams to understand participant needs and help implement practical support strategies. You will assist...
