The Intensive Support Team Calderdale (ISTC) is seeking an enthusiastic and autonomous Band 5 practitioner on a full-time basis. Referral Considerations

• Onset of relapse or severe mental distress requiring a rapid, flexible, and intensive response.
• Brief interventions to augment or support existing care plans.
• Assessment when hospital admission is being considered or a gate‑keeping assessment is required.
• Support for early discharge or graded leave.
