We are looking for a Senior Nurse, experienced working with patients in the community with complex long‑term conditions. You will join our North Kerrier Community Matrons who, as part of the wider Community Nursing Team, work closely across Adult Community Services, Adult Social Care and Primary Care. Using enhanced clinical skills we enable patients to remain independent at home for as long as possible and reduce the risk of avoidable hospital admissions.

The role requires completion of the HEAB 358 Advancing Practice in Long Term Conditions Management module or the equivalent, and the post holder should be an independent prescriber or working towards this qualification.

The post holder will need a high level of communication, problem‑solving and decision‑making skills.
