Make an impact in community health as a Registered Nurse with Vancouver Coastal Health in North Vancouver. This role requires proactive engagement and support for diverse patient needs.
As part of the Home Health team, you’ll assess patient eligibility and create care plans that promote independence among clients with chronic health conditions. Utilize your clinical expertise within a supportive and collaborative environment to enhance care delivery.
Key Responsibilities:
• Conduct assessments to determine client service needs
• Develop, implement, and adapt personalized care plans
• Facilitate access to community resources and support

Requirements:
• Registered Nurse with current BCCNM registration
• One year experience with complex health issues
• Must have a valid BC Driver's License
• Knowledge in community nursing and care models
