Clinical Pharmacist

Summary VA Ann Arbor Healthcare System (VAAAHS) is seeking a full time Clinical Pharmacist to work at their Toledo, OH Community Based Outpatient Clinic (CBOC). The clinical pharmacist is a licensed professional with knowledge and proficiency in pharmacy practice including pharmaceutics, pharmacokinetics, pharmacodynamics and pharmacotherapeutics. Responsibilities Major duties of this position will include but are not limited to: Provide appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; Advises prescribers as appropriate. Provide patient-specific therapeutic drug monitoring and communicates relevant findings and/or recommendations to other health care providers in charge of the patient both orally and in writing via notes in the electronic health record.
