Clinical Care Coordinator

We are seeking an experienced Clinical Care Coordinator to join a dedicated team in a beautiful, popular coastal holiday destination, approximately four hours from Melbourne. This is a part-time role with the fantastic opportunity to increase your hours to full-time by taking on Registered Nurse (RN) shifts. The successful candidate must possess:

• Current AHPRA registration.
• Full working rights in Australia.
• Proven experience in a coordination or team leader capacity within a clinical setting.

In addition to the mandatory AHPRA registration, work rights, and experience, the role requires:

• A strong understanding of the Aged Care Quality Standards.
• Knowledge of the common physical, psychological, and social issues affecting frail older people and those living with dementia.
