Job Summary

Siloam Mission’s Medical Transitional Care Unit (MTCU) is a compassionate 32-bed facility supporting individuals experiencing homelessness who have medical needs that do not require hospitalization. The MTCU provides time limited transitional housing integrated with essential medical and social supports to improve health and housing outcomes. Guided by person-centered, strengths-based, trauma-informed practice, and rooted in harm reduction and Housing First principles, the MTCU operates through a holistic and multidisciplinary approach. The MTCU Case Worker provides case management, medical system navigation, and housing support to residents in the unit. Working collaboratively with the Intake and Transition worker, residents, clinical staff, and community partners, the Case Worker supports individuals to navigate health care and social systems and successful transition to permanent housing.
