This role includes providing guidance and training to Mentors, maintaining consistent communication with the team, and relaying information between Case Managers and Mentors. The Caseworker assists with coordinating community supports and completes duties assigned by the Case Manager to ensure program activities align with participant needs.Additionally, the Caseworker will support participants with day-to-day needs and provide transportation to appointments such as doctor visits, EIA meetings, and other required services.
