Role SummaryWe are seeking a Registered Nurse (RN) to support part-time, interim basis. This role focuses on care coordination, clinical support, and interdisciplinary collaboration across multiple skilled nursing facilities.
This job will have the following responsibilities:
Ensure accurate and complete clinical documentation Collaborate with interdisciplinary care teams and providers Support care coordination and patient management activities Assist with Medicare (Parts A, B, D) benefit determination Monitor and respond to changes in patient condition Coordinate therapy services and rehabilitation care
