SUMMARY: The Case Manager III provides assessment, monitoring, planning, linkage, and advocacy for the most appropriate services to individuals enrolled in the Supportive Services for Veteran Families Program. Qualified candidate must possess a clear understanding of the service delivery system and the resources available for veterans and their families. Individual will effectively maintain and manage assigned caseload contingent on needs, strengths, abilities, and preferences of the individual served. This position will be located in San Juan, PR.
