Join TNG as a Mobile Shelter Support Program Case Manager in a full-time role. Provide critical support to vulnerable adults in shelters and other facilities across the Greater Toronto Area.
As a Case Manager, you will work directly with clients facing complex issues such as health, housing, and legal difficulties. Your role will involve conducting assessments, coordinating support services, and fostering partnerships with external providers. Utilizing a harm reduction approach, you will empower participants to reach their self-determined goals and ensure access to essential community resources.
Key Responsibilities:
• Conduct intake and assessment interviews with participants
• Monitor property issues and coordinate with landlords
• Connect clients to community supports and services
