Job Summary:

The Coordinator is responsible for developing and supporting an individual support plan for clients with developmental disabilities in receipt of residential support from the Family Home Program. These services are funded by the Ministry of Children, Community and Social Services. The Coordinator reviews and assesses the needs of individuals requiring support related to all aspects of their support plan.
