Care Partner

As a Care Partner, with our Resilience team, you’ll guide customers through their care journey with compassion. Working closely with families, clinical teams, and external providers, you’ll develop holistic health plans, coordinate services, and ensure customers receive safe, person-centred care that enables them to age well at home or in a place of their choosing. This is a virtual role, with all customer support provided over the phone. 

This is a hybrid opportunity with the option to work 2 days from home and 3 days from our Mackay branch.
