We have an exciting opportunity for a Care Partner to join our Home Care Self-Managed (SMD) team offering telephone-based care and virtual care for our clients.

As this is a telehealth position it will offer a hybrid working model of office and WFH arrangement and could be based in Clayton, Victoria.

This position will be in our self-managed care teams. We cater our support to each client’s unique care needs to ensure we have the best solution for every Home Care Package holder.
