Reports To: Community Connect Program SupervisorFLSA Status: Non-ExemptProbation Period: Three (3) MonthsLocation: Varies Position Summary Job Summary Under the direction of the Community Connect Program Supervisor, the ECM Care Manager delivers direct services and care coordination as part of a multidisciplinary team.
You will assess whole-person needs, develop and execute care plans, advocate for clients, and ensure timely access to health and social services that drive wellness, recovery, independence, and empowerment.
This position is Hybrid and will be in-office only for the first 90 days.
Essential Job Responsibilities Assess client needs across physical health, mental health, SUD, oral health, trauma history, social supports, housing, employment, and community services.
Develop and oversee Individual Care Plans (ICP) and Health Action Plans (HAP).
Provide services in-office, via telehealth, and in the field--meeting clients where they are.
