Patient Care Coordination

• Enroll eligible patients into Care Management and Chronic Care Management (CCM) programs.
• Conduct regular outreach to patients via phone, telehealth, or secure messaging.
• Assist in developing, updating, and maintaining individualized care plans under provider direction.
• Monitor patient progress, adherence to treatment plans, and care goals.
• Identify care gaps and escalate clinical concerns to licensed providers as appropriate.

Care Management Support

• Coordinate referrals, follow-up appointments, labs, and diagnostic tests.
• Facilitate communication between patients, providers, specialists, and external care partners.
