Join our growing Ageing Services Team as a Care Coordinator and make a real difference in the lives of older Australians.
This is a full-time, 12-month contract, with the possibility of extension, where you’ll empower clients to live with dignity, independence, and connection. You’ll manage a caseload of Commonwealth Home Support Program (CHSP) clients, ensuring their care journey is seamless and meaningful. Your role will involve coordinating holistic support services, conducting assessments and reviews, and building trusted relationships with clients, families, and providers.
