As a Care Coordinator, you are the heartbeat of our operation—the bridge between our providers, our technology, and the people we serve. We're looking for an advocate, a problem‑solver, and a steady hand for our members. You will manage the end‑to‑end journey of our clients, ensuring they feel seen, heard, and cared for at every touchpoint.

Job Type

Full-Time (40 hours/week)

Schedule

Flexible roster. Includes a mix of standard business hours and some rotating weekend shifts when needed, to ensure our members are supported 7 days a week.
