Provide a short description of the Position: OHCCT supports hospitals to strengthen Mental Health admission, treatment, and discharge practices and related outcomes in OMH licensed Inpatient Psychiatric Units and Comprehensive Psychiatric Emergency Programs (CPEP), and Emergency Department (ED).Tracking care transitions between inpatient, CPEP, EDs, residential and ambulatory levels of care, foster positive and supportive relationships between OMH, hospitals, community-based agencies and identify/track capacity and demand of services available. The Center for Human Resources Management (CHRM) Portal allows for the Office of Mental Health agency and human resources to examine key elements of established positions and their incumbents as of a selected effective date, using data from the Department of Civil Service's New York State Electronic Personnel System.
