Behavioral Support Lead (Dr. Paul Woo Long-Term Care Centre)

Position Summary

The Behavioural Support Lead serves a dedicated role as an in-house Lead on identification and management of responsive behaviours associated with dementia, complex mental health, substance use, and related neurological conditions. The person in the role will lead the Home to work with the Behavioural Support Team (BST) hospitals, primary care providers, and other providers and professionals. The Lead will participate in Central LHIN’s Behavioural Support Services Program.

Main Duties

• Provide leadership and support to Home in the management of responsive behaviours.
• Maintain an active clinical caseload of residents with responsive behaviours.
• Convene and lead regular Behaviour Rounds to enable front line staff to discuss care plans with internal and external resources.
