We are offering a permanent full‑time position with the South Adult Social Care Occupational Therapy Team at Band 4. Previous Adult Social Care or Community Occupational Therapy experience will be advantageous.

You will work as part of a multidisciplinary team of Occupational Therapists, Assistant Practitioners and Assistants to provide care to adults with a range of conditions across a variety of specialities.

Main duties of the job

Support the delivery of rehabilitation, education and prevention, and self‑management/lifestyle advice for long‑term conditions. Aim to maximise function, prevent deterioration and enable safe discharge and/or follow‑up in the community with short‑ and longer‑term rehabilitation programmes.
