Advanced Practitioner in Social Work

An exciting opportunity has become available for an Advanced Practitioner in Social Work to work within the Prevent, Reduce and Enable Service, based within the East Adult Contact Team at Macclesfield Town Hall. The position is full time 37 hours a week, from Monday to Friday.

About the role

This position supports the Adult Social Care Teams in the East on the Prevent, Reduce and Enable Team, providing a quick and urgent response to individuals referred with a presenting need. It is a fast‑paced role working alongside a small team of Social Care staff, Occupational Therapy and the Reablement Service. The aim is to prevent further crisis or deterioration in needs and to prevent hospital admission. The Advanced Practitioner ensures practice standards are maintained; assessments are strengths‑based and person‑centred; interventions are proportionate to presenting needs; and staff receive guidance and supervision to tackle challenging conversations and conflict.
