Advanced Nurse Practitioner

We are seeking an Advanced Nurse/ Clinical Practitioner to join our busy practice on the Eastern side of Northampton.

Our ideal candidate will be an experienced prescriber with an interest in one or more longterm conditions as well as the ability to assist our GPs triage minor illness calls.

Our practice is patient centric and keen to ensure patients get the best treatment to meet their needs.

Main duties of the job

You will be expected to:

Triage patients withundifferentiated and undiagnosed problems and make an assessment of theirhealthcare needs

Undertake assessment, diagnosis, treatment,prescribing, referral and discharge as appropriate and deliver key elementsof NSFs, providing public health, health protection and promotion programmesthat improve health and reduce inequalities

Work with the clinical team to achieve maximum QoF outcomes.
