About the Role
We are seeking an experienced and compassionate Advanced Clinical Practitioner (ACP) to join our Special Allocations Scheme (SAS) service.
The SAS service provides essential primary care support for patients who have been removed from mainstream GP practices due to challenging behaviours. The service operates Monday to Friday between 08:00 and 18:30hrs.

Key Responsibilities
- Provide first-contact assessment, diagnosis, and management of undifferentiated physical and mental health presentations
- Deliver remote triage and face-to-face consultations using advanced clinical assessment and decision-making skills
- Manage acute presentations, long-term conditions, and preventative healthcare interventions
- Prescribe safely and appropriately within scope of practice
