Adult Practitioner

Location: Telford Area Pay Rate: £16.35 per hour PAYE Hours: 37 hours per week, Monday to Friday, 9:00am 5:00pm Contract: Temporary until 31 August 2026 We are currently recruiting for an experienced Adult Practitioner to support a busy Adult Social Care team. This role offers a hybrid working arrangement, with office attendance required on Mondays and a combination of home working and client visits throughout the week. Key Responsibilities Complete Care Act assessments, reviews and reassessments. Carry out Carers Assessments and Mental Capacity Assessments. Develop strengths-based, person-centred support plans. Promote independence, wellbeing and informed choice. Support access to community resources, assistive technology and digital solutions. Manage risk and contribute to safeguarding activities.
