Position Summary

Working as a member of the interdisciplinary Community Wellness team, the Social Worker provides a diverse range of client care to ensure client engagement and to reduce or remove barriers. This care includes extended community outreach, mental health support, health education, case management, treatment support, counseling, community programming, and advocacy for clients with acute and complex care needs.
